1. Number of copies of application documents to be submitted
   Applicant needs to submit one copy each of the required documents.

2. Commission of application
   In case that application is performed by a corporation or an organization, the applicant shall be its representative, and seal impression of the responsible person shall be used. When application procedure is entrusted to other person, a letter of attorney from the responsible person to the mandatory is required. In such a case, seal impressions of the responsible persons of both the applicant and the mandatory shall be affixed to the application.

3. Submission of applied equipment
   Applied equipment should be submitted to TELEC office. However in case of the following (1) or (2), the equipment may be submitted to a place other than TELEC office;

   (1) when it is difficult for TELEC to keep the equipment in its premises because of a great volume of equipment, or
   (2) when TELEC accepts the request from applicant to conduct certification work at a place other than TELEC office.

4. Attachments for test
   Applicants are requested to submit attachments such as cables and/or connectors to enable smooth connection between measuring instruments and applied equipment.
5. Quantity limitation

   In case of the application for test certification, the number of radio equipment under one application is limited to 500 units or less. This is not applied for type certification.

6. Acceptance of application documents
   TELEC examines the format of the application and the attached documents, and accepts them if they are correctly made.
